I focused on creating a clean, minimalistic form that simplifies the referral process while maintaining TimTim's professional image.

Nice start! I think maybe adding some sort of human element to the design would help this connect to workers of TimTim. Using maybe a stock photo of retail employees instead of the imagery you have currently could help make the intake form more inviting. Also maybe adding more of their secondary accent orange color could help as well. - happy Designing

Thank you for the feedback! I think that's a great idea. Adding a stock photo of retail employees and more of the orange accent will definitely make the form more inviting and relatable. I'll work on refining it!
