this is a refer a friend form which contains the name, email address and phone number as well as additional information and is also colored based on the company's request.

Simple and concise! My only comment is that for the vertical alignment for the input in the message box should be top-aligned rather than middle :)

Nice clean design. The input fields could look a little more modern. I would recommend lightly rounded corners and removing the stroke except on the active field.
